Continuity of Care
Your doctor works closely with nurses and other qualified health professionals to better manage your healthcare.

General Practice is a long-trusted healthcare model that provides community-based, family medicine as the “first line of contact' in our healthcare system.
Our general practitioners (GPs) are doctors who work on behalf of the community to improve the health of patients and families in your own neighbourhood. Working with nurses and other professionals, our GPs take care of you when you are ill or injured, and help you prevent illness or manage long-term health issues.
If you suffer from long-term health issues - such as diabetes - some of our practices offer nursing clinics where you can arrange regular check ups on your condition.
Our qualified doctors and nurses also serve as your advocates and gatekeepers in the wider healthcare system. If they can't deal with your health issue at their office, they know exactly whom to refer you to for help. You might be referred to hospital-based, community or private providers, such as:
• Diagnostic services like x-rays, MRIs and blood tests
• Mental health specialists
• Hospital or secondary-care specialists such as cardiologists, opthmalogists and oncologists
• Speech language therapists
• Occupational therapist
• Midwives
• Plunket nurses.
Each of these providers are in regular communication with your practice GP, helping to oversee all your health needs.
