ProCare works closely with individual communities to develop healthcare services and programmes that meet the needs of Auckland's diverse population -including Maori, Pacific and Asian.
Community Health Coordinators
ProCare community health coordinators help patients access healthcare and social services. They coordinate healthcare services with doctors, hospitals and other health professionals; advocate for you with WINZ and Housing New Zealand; promote immunisation and health services; and contact patients for follow-up care.
Community-specific programmes
ProCare GPs and nurses serve more Maori, Pacific and Asian patients than any other primary healthcare organisation in New Zealand. We look after your whanau and community by providing a range of unique programmes, such as:
Maori Health initiatives:
- Nga Rima o Kaipara Marae Arataki - Award-winning holistic health promotion programmes at five marae in South Kaipara.
- Marae Hauora programme - Marae Lifestyle coordinators through Huakina Development Trust promote healthy lifestyles at 10 marae with range of organised activities.
- Whare Oranga programme - This programme provides a full suite of GP, nurse-delivered medical services, the Community Health Coordinator services and other healthcare services on three marae in Te Puaha o Waikato and Franklin.
Pacific Health initiatives:
- Healthy Village Action Zone -Providing parish nurses, Community Health Coordinators, and health promotion activities in 42 Auckland churches (in partnership with ADHB).
- Big Boys - Big Girls Club - Helping overweight patients in Manukau.
- Parish nurse in Mangere - Working with churches around the area.
- Integrated Pacific Health initiatives - Healthcare services across West Auckland and North Shore.
South Asian and migrant communities:
- Culturally appropriate health promotion for South Asian communities -Includes diabetes, nutrition and Bollyrobix, an aerobics class set to Bollywood music.
- Muslim women’s swimming programme - Addressing the unique recreation and social needs of more than 150 new immigrants and refugees living in Mt Roskill area.
