The cooperative group of Auckland agencies working in this field want to learn what people think about Advanced Care Planning - which is the term used for pre-planning people can do with their GP about the care and treatment they want at the end of their life.
Below is a link to a survey which will take around 8 minutes to complete. We would appreciate your feedback on Advanced Care Planning, as a way to enhance our services to our patients.
